Required Forms

Additionally you will need to submit:

• Q4 2024 report card 
  
• New players will need to submit a copy of an official birth certificate

1. Download the forms you need from this page, fill them out, and save them
2. Log into Sports Connect
3. Make sure you are on the My Account section. You should see a list of players with your child listed and their information
4. Click on the pencil next to their name
5. Next to the profile picture is where you can upload the birth certificate
6. Scroll down to find the place to upload the Medical History, Player Contract, Report Card, and Scholastics form]

* Unfortunately the system doesn't always send the paper work to the national system.  If you are having issues we can upload the paperwork for you
