Forms:
1. Copy of Birth Certificate (original will only be needed if the team/squad continues to play-offs)
2. Player Participation Contract (can be found by clicking on the link below. It is located in the 2010 forms and is 2 pages. Both parent and player will need to sign the back form. Please make sure to include your email address if applicable)
3. Pop Warner Physical Form (can be found by clicking on the link below. Physical has to have been completed since Jan. 1st, 2010. The Panthers will be scheduling $15 physicals in June/July. Please check back for details.)
4. Copy of the 2009/2010 FINAL report card.
5. $80 Registration Fee. (Sibling discount is $10 so two siblings will be $140, three is $210)(scholarships are available. For more information or to apply for a scholarship, please contact Popwarnerpanthers@gmail.com)*Fee increases to $90 per participant after Aug. 1st-no sibling discount*
These forms can be turned in at Play It Again Sports on June 26th or July 17th from 10 a.m. - 1 p.m.. You may also mail your registrations to: Pop Warner Panthers Registration, 604-2 Tennessee Ave, Carolina Beach, NC 28428. Forms may be turned in at the first day of practice, but please aware that the participant can not practice until all forms are in and the lines on Aug. 2nd can be very long.
